The COVID-19 vaccine is now available for any individual over the age of 18. Vaccines available April 23th at the Platte Valley Community Center from 10 a.m. – 2 p.m. No appointment needed.
To prepare for your COVID-19 Vaccination, please:
Print off, fill out, and bring our vaccination release form and questionnaire the day you go to get your vaccine. You can obtain a copy of the form by visiting https://health.wyo.gov/wp-content/uploads/2020/12/COVID-Vaccine-Consent-Form-Template.docx
Copies are available at the Carbon County Senior Center for their patrons.
Please Bring your Insurance Card: We are able to bill insurance for the administrative fee, but the vaccine is available for free at no cost to any individual regardless of insurance coverage.
Please wear your face covering throughout the clinic and try to maintain 6 foot social distancing to protect yourself and others.
Participants should not attend the clinic if they are feeling sick, have a fever, or are exhibiting respiratory symptoms or have been diagnosed with COVID-19 in the past two weeks.
Participants should wear clothes that allow the immunizer easier access to administer the shoulder area for a more efficient vaccination exposure.
It is not recommended to take any NSAIDS, Tylenol or Benadryl prior to receiving the vaccine, these medications should be reserved for if you need them after receiving the vaccine.
